Payment and Policies
I currently accept the following insurance carriers:
Aetna (commercial plans only- I do not accept Aetna Better Health)
Anthem, Blue Cross/Blue Shield (all plans)
Cigna (all plans)
Humana (commercial and Medicare plans only- I do not accept Tricare/Humana Military)
Magellan Healthcare (all plans)
Medicare ('straight' Medicare plans as well as several others)
Optima (all plans)
United Healthcare/Optum (all plans)
Please contact your insurance company to verify your benefits, to find out if you need to do anything prior to scheduling an appointment to be covered, and how much your co-pay is per session.
Payment is expected at the time of service: I currently accept cash, check and all major credit cards for co-pays and self-payments.
I offer sliding scale services on a case by case basis to individuals experiencing financial hardship.
If you need to cancel your appointment, please let me know at least 24 hours in advance. If you cancel your appointment with less than 24 hours notice you will be expected to pay a $100 fee, which you will be billed directly for as insurance does not cover no-shows or late cancellations.
